Better Health with Self-Management

“Please conduct this and other courses again they are great as they also get people to stop hibernating and get out there and interact with others.”

Chronic Disease such as diabetes, asthma, bronchitis, emphysema, arthritis and heart disease to name a few, contribute to over 70% of the disease burden in Australia and this figure is expected to increase.

The Australian National Chronic Disease strategy identifies Self Management as one of four key action areas and is striving to strengthen the capacity of individuals to self manage more effectively. This is particularly important in our older population who often tend to be passive recipients of health care rather than active participants.

The course teaches participants the life skills needed in day to day management of Chronic Conditions, and to embrace life's opportunities. It has been designed to enhance regular treatment and disease specific education and will not conflict with existing programs or treatment. It is targeted to those that live with Chronic Disease, their carers or support people, and those at risk of developing Chronic Disease. The program was developed by the school of medicine at Stanford University and has been life-changing for many people. The Stanford model is renowned as one of the best courses worldwide and over the past ten years has been delivered statewide by Anglicare Southern Queensland.

“I have really loved this course and the group were great. It helps me to meet others in the same boat.”

The two and half hour work shops run once a week, over a period of six weeks and is highly participative. The format reduces and individual's sense of isolation, facilitates peer learning and sharing, and has a strong goal setting and problem solving focus. The aim is to promote and build participant's confidence, empowering them to manage their health, and therefore maintain active and fulfilling lives. Studies have shown self managers have more effective relationships with friends, family, their general practicioners and other health professionals resulting in enhanced health outcomes, increased independence, reduced hospitalisations and subsequent improved quality of life.

Each week clients are provided with a great deal of information and resources including State and Federal initiatives.

You will be assisted by two trained facilitators over the 6 weeks. The sessions are extremely supportive and enlightening. Each week you will be encouraged to take on new learning's, apply the key concepts and most of all improve your health outcomes. Come along, challenge yourself and reap the rewards.

“Have not done anything in the area for 25 years. So many new pieces of information and care options.”

“I have learned so much, grown so much and added so much more to my quality of life.”
